We regularly see lead forms asking for far too many questions, and in our experience, the best converting forms often only request two or three pieces of information: name, email address and telephone number.

For example, include your customer testimonials on this page, include logos of the review sites that you are on (for example, Trustpilot or Google Reviews).
And our final tip would be to treat the leads like gold dust and to follow up faster than your competitors.
